RESUMO

Subcutaneous emphysema occurs when air is forced under the tissue, causing swelling, crepitus on palpation, and the possibility of spreading along the fascial planes. Although subcutaneous emphysema secondary to dental procedures is rare, it can be a potentially fatal complication if not diagnosed and treated promptly and correctly. Dentists need to be able to differentiate subcutaneous emphysemas from more common disease processes that have similar clinical presentations. We report a 22-year-old male who underwent mandibular wisdom tooth extraction and subsequently developed extensive subcutaneous emphysema. The patient was quickly taken care of, in partnership with the maxillofacial department. The purpose of this report is to bring attention to the fact that obtaining an accurate diagnosis for this condition is very important and management on time can prevent serious complications.

RESUMO

Myxoma is a rare variety of benign tumors of the jaw, whose origin is embryonic mesenchymal dental follicle. Clinical and radiological manifestations are variable and non-specific and can lend confusion with other lesions. We describe a case of very invasive odontogenic myxoma , observed in a woman of 51 years. Clinically, the patient had left maxillary swelling, painless, firm to the touch. CT facial bones showed an expansive osteolytic process blowing the jaw. The treatment consisted of a wide excision of the tumor by endobuccal way, the tumor is individualized well from the bone. Pathological examination of the surgical specimen was diagnosed odontogenic myxoma. The short-term trend has shown no clinical and radiological local recurrence. The diagnosis is based on clinical, radiological and histological. This tumor is locally aggressive, which can lead to significant facial deformities and dental disturbances. The recurrent nature of myxoma imposes a broad radical treatment beyond the limits of the lesion. The loss of important substance required surgical repair or prosthetic.

RESUMO

First described by Philipsen in 1956, the odontogenic keratocyst is characterized by a large squamous keratinization of its border, an aggressive growth and a high recurrent rate. It is now designated by the World Health Organization as a Keratocystic Odontogenic Tumor (KOT). Clinically, the KOT is manifested by an asymptomatic growth. Radiographically, it appears as a well-defined unilocular or multilocular osteolytic lesion. The diagnostic approach is based on a combined analysis of the medical history, the clinical appearance and the radiographic appearance. The diagnosis may be confirmed by the anatomical pathology report. Finally, treatment consists of surgical excision and follow up is characterized by a high rate of recurrence. The authors report a case of keratocystic odontogenic tumor of the mandible and review the various diagnoses, therapeutics and follow up aspects of this type of tumors.

La biopsie est un examen complémentaire d'une valeur diagnostique primordiale, permettant de confirmer ou d'infirmer le diagnostic posé lors de l'examen clinique. C'est un acte chirurgical qui requiert une technique rigoureuse de la part de l'opérateur.Notre travail se propose donc de décrire, de façon détaillée, les indications et contre-indications de cet acte opératoire, ainsi que le protocole opératoire des biopsies les plus fréquemment réalisées en pratique courante, à savoir, la biopsie incisionnelle, excisionnelle et biopsie des glandes salivaires accessoires

RESUMO

L'hygiene au cabinet dentaire est beaucoup plus une question d'organisation; d'ergonomie et de coherence gestuelle que de cout. L'objectif de ce travail est de faire une evaluation du cout de l'hygiene et de l'asepsie au sein d'un cabinet dentaire au Maroc; de demontrer que ce cout est gerable et de proposer des solutions de gestion. La 1ere partie de cet article fait le point sur tout le materiel necessaire pour une consultation; un soin ou une chirurgie ainsi que le cout proprement ditLa methode d'evaluation du cout a pris en consideration le cas de figure d'un praticien recevant 15 patients par jour et travaillant 5 jours par semaine; ce qui fait un total de 3300 rendez-vous par an. Il a ete estime que les actes chirurgicaux representent 25de la totalite de l'activite. L'application de l'ensemble des procedures d'hygiene et d'asepsie suivant les normes; les recommandations et les reglementations en vigueur s'eleve a un cout annuel estime a 125333 dhs/an. Ce montant ainsi que la complexite d'application des mesures d'hygiene nous ont amenes a poser plusieurs interrogations. C'est ainsi que des suggestions basees sur une approche de gestion et de management seront proposees afin d'ameliorer le niveau d'asepsie sans pour autant augmenter les couts

La gestion des dechets des activites de soins est donc capitale; elle doit respecter les cinq etapes fondamentales de tri; de conditionnement; de collecte; de stockage; de transport jusqu'au traitement lequel se fait soit par incineration soit par enfouissement
